It looks to be a machine to wind thread onto a bobbin or spool.

The thread would go in the slotted holes at the top and bottom of
the arm thing and the gear with the cam moves it from side to side
while the knob on the upper right turns it.

Looks like it's missing the handle and a screw in fitting at the end of the
swivel arm... My guess is it's probably some sort of turn of the century
rivet or eyelet applicator.

That's a 1983 liberty quarter coin (25 cents).

Strictly speaking the edge of the quarter is "milled" not "geared"
though I understand that they could appear similar to a layperson.

The object that you have put with the quarter for size comparison is
called a reloader. It squeezes lead bullets on the end of cartridges.

It's part of the inker from a Multilith 1250.
The 1250 is (was) an offset duplicator used in print & copy shops.
